Metformin Is Still the Cheapest Tool in the Box

Metformin remains the first-line drug for type 2 diabetes in the United States, and it is also the cheapest. A month of generic metformin 500 mg tablets often runs $4 to $10 at Walmart, Costco, or a Kroger pharmacy counter, even without insurance. If you want a 90-day supply, many chains price it around $10 to $15. The extended-release version, sold as metformin HCl ER, is slightly more but still usually under $20 a month at cash prices.

That low price matters because the drug works. Metformin lowers the amount of glucose your liver releases and helps your body respond to insulin. The most common complaint is stomach upset, especially in the first two weeks. Taking it with food and starting at a low dose usually fixes that. The ER form is gentler and is often prescribed when the regular tablets cause diarrhea.

One thing people miss: metformin is not a weight-loss drug. It may help slightly, but the effect is small compared to GLP-1 medications. If your doctor talks about a brand-new type of diabetes treatment, they probably mean semaglutide or tirzepatide, not a reformulated metformin.

The Insurance Question You Should Ask First

Before you fill any prescription, ask your pharmacist to run a test claim through your insurance. This costs nothing and tells you the real out-of-pocket price. Many people assume a drug is covered and then get a surprise bill. The test claim saves that headache.

Ozempic, Semaglutide, and the Aetna Coverage Maze

Semaglutide, sold as Ozempic for diabetes and Wegovy for weight loss, has become the most talked-about drug in American medicine. The list price for a month of Ozempic is roughly $900 to $1,000 without insurance. Wegovy is similar. That number is why so many people ask whether Aetna covers Ozempic.

The short answer: sometimes. Aetna and most large insurers cover Ozempic for type 2 diabetes when a doctor documents the diagnosis and prior authorization is approved. Coverage for weight loss is much narrower. Some Aetna plans exclude weight-loss drugs entirely, while others cover Wegovy only if you meet a body mass index threshold and have tried other options. Medicare Part D generally does not cover drugs for weight loss alone, though it can cover them for diabetes or heart disease.

If you are paying cash, the manufacturer’s patient assistance program and discount cards can bring the price down, but rarely below a few hundred dollars a month. Some people use compounded semaglutide from a licensed pharmacy as a cheaper option, though the FDA has warned about unapproved versions. Talk to your prescriber about what is legal and safe in your state.

How Much Semaglutide Should I Take?

Dosing is not something to guess. Ozempic starts at 0.25 mg once a week for four weeks, then moves to 0.5 mg, and can go up to 2 mg under a doctor’s direction. The starting dose is designed to let your body adjust and reduce nausea. Skipping ahead does not speed up results and often makes side effects worse. Your prescriber sets the schedule, and it should be followed exactly.

Red Meat, Sausage, and the Diabetes Conversation

Diet questions come up at almost every diabetes appointment, and the one about red meat and diabetes type 2 is fair. Large studies, including long-running cohorts of U.S. nurses and health professionals, link regular consumption of processed meats like sausage and bacon to a higher risk of type 2 diabetes. Unprocessed red meat shows a smaller but still positive association. The mechanism is not fully settled, but saturated fat, sodium, and preservatives all play a role.

That does not mean you can never eat a hot dog. It means frequency and portion matter. A practical rule many dietitians use is to keep processed meat to once a week or less and to pair meals with vegetables, beans, or whole grains. Swapping one serving of red meat a day for chicken, fish, or legumes is a realistic change that shows up in blood sugar readings within a few months.

Night sweats are another common complaint. They can come from low blood sugar overnight, from menopause, or from unrelated causes. If you wake up drenched and your morning glucose is low, talk to your doctor about your evening medication dose. If it happens with normal glucose, keep a log and bring it in. Patterns are easier to treat than one-off events.

Getting the Most From Your Pharmacy Dollar

Prices for the same drug vary widely between pharmacies. A metformin prescription that costs $4 at one counter can cost $30 at another. Call three pharmacies near you and ask for the cash price and the insurance price. GoodRx and similar discount cards often beat insurance for generics, and they are free to use.

Ask about 90-day fills. Most insurers charge one copay for a three-month supply, which cuts your yearly cost by a third. Mail-order pharmacies through your plan usually offer the same deal. For anyone on a high-deductible plan, this is the single easiest way to save.

Finally, check the formulary every January. Insurance plans change drug tiers each year, and a medication that was $30 in December can jump to $200 in January. A five-minute call to your plan’s member services line in early January prevents a lot of sticker shock.
